A Clinical Study of 140 Cases of Cholelithiasis


Abstract
140 consecutive cases of cholelithiasis treated surgically in, Ewha Woman¡¯s University Hospital from Jan. 1965 to Dec. 1974 were studied. There were several differences in their manifestations contrast to report in the western literature, that is, as is common to many races of oriental countries, bile duct stones are common in Korean than European.
Author believed that these differences should be properly reflected in the treatment of these people by modifying certain therapeutic principles.
Some of the important findings and suggestions are as follows:
1. The highest incidence of cholelithiasis was found in, the 4th decade and the
sex ratio was 1 male to 1. 26 -female.
2. The majority of the patients were operated within 1-2 years after the onset of the diseases.
3. The most prominent symptoms was pain (94.3%) in the R.U.Q., and epigastric region.
4. In laboratory data, leucocytosis was noted in 70%, On cholangiographic analysis in 72 cases, 58 cases were not visualized in the gallbladder and 14 cases showed normal function of gallbladder with good visualization of the gallbladder.
5. In the location of cholelithiasis of 80 cases, 37 cases showed stones in the gallbladder, alone 68 in the common bile duct, 26 in the both side, 6 in the CBD and IHD, and 3 in GB, CBD and IHD.
6. The total number of cholecystectomy was 38 cases, and cholecystectomy with choledochostomy¢¥ 91 cases. Relatively high % of common duct ei ploration reflects high incidence of common bile duct stones.
7. The postoperative complications were 26 cases, in which the wound infection was most frequent and its mortality rate 0. 7%.
